Well Planned Action Series: An Opportunity for Multidisciplinary Teams

The Well Planned Action Series is a learning opportunity for small multidisciplinary teams. It consists of five theme-based video modules and activity guides that can be completed asynchronously or together. It was created locally by physicians, nurses, allied health professionals, and support staff from across the Island Health region to support teams to improve the experience of people creating, clarifying, and communicating medical goals for discharge. Each area has unique challenges and the best people to identify opportunities for improvement are the ones who are doing the work.
 

Many factors influence the ability of all team members (including patient/family) to be aware of a patient’s medical goals for discharge. The Well Planned Action Series will support teams to explore which factors affect them and help identify high impact-low effort ideas for improvement they want to test in their area.
 

Medical Goals for Discharge provide information about the clinical reason for a patient’s admission and describe the clinical status the patient and team are working towards in order to be medically fit for discharge. Medical goals for discharge is not a checklist to be achieved before discharge (criteria led discharge), or a component of advanced care planning (goals of care).

Action Series highlights:

  • Flexible time commitment, up to 6 hours spread over 5 months. Approximately 10-20 minutes of videos and 45 minutes of team activities per month. Videos and activities can be done in as little as 5-10 minute segments. Up to 6 hours of sessional payments at the JCC rate is available for physicians completing activities outside of clinical hours
     
  • Learn tools and techniques your team can try right away and can apply to other problems in the future for grassroots level change 
     
  • Participate in multidisciplinary teams of 3-7 people – pleaseinclude at least one physician and one non-physician role, including Nurse Practitioners, Midwives, Nurses, Allied Health professionals, etc.

Example team compositions:

  • 3 RNs, 2 CNLs & 2 Physicians
  • 2 NPs, 1 Physician & 2 LPNs
  • 1 Physician, 1 CNE & 1 SLP
     

The Well Planned Action Series is running November 4th, 2024 to March 30th, 2025 (check out the Timeline for more details)

Want to join but aren’t sure about building a team? We recommend asking two friends you work with to try it with you! There is no Team Lead  role and no additional responsibilities for being the person who pulls a team together. Please reach out to PQI@islandhealth.ca if you have any questions or if you need help identifying a team.
