By the end of Module 2, you will be able to:

  • Describe the roles, skills, and workload within the care environment of each of the team members
  • Identify the processes involved in creating, communicating, and clarifying medical goals for discharge from each perspective
  • Identify potential opportunities for improvement within sphere of control

Each Module contains a set of videos and an Action Period Guide. While you can do these in any order, we recommend following our “playlist” below and then completing the Action Period Guide activities with your team members.

Action Period Guide for Module 2

Required Activities:

Understanding the different roles and perspectives of your Action Series Team members, and what parts of the process they have sphere of control and sphere of influence over, will help you identify opportunities for improvement that you can test change ideas on during the later modules. The Action Period Worksheet will guide you through two great activities.

For change to be successful and spreadable, you need to start by developing awareness with your broader team. Share what you are doing early and often. The monthly Team Activity Update will help you do that!
