Complete the Pre-Action Series Team Assessment Survey
Please take 1-2 minutes to complete the Pre-Action Series Team Assessment Survey as soon as possible, using your Team Name, not your own, to ensure anonymity. Answer each question about your Well Planned Action Series team from your perspective. Each team will get a report with their results consolidated as an average.

For Medical Staff who would like to continue to build their Quality Improvement skills, PQI offers free access to the Institute for Healthcare Improvement (IHI) Open School, as well as locally taught QI Fundamentals and a year-long QI Cohort training program (sessional funding is available for physicians). Email PQI@islandhealth.ca to get notified of upcoming opportunities, to request access to the PQI Group Subscription for the IHI Open School, or for information on sessional funding.
