Substance Use Care Plan

WHAT IS HAPPENING

Island Health has launched the Substance Use Care Plan across all acute care sites effective immediately. This care plan provides a standardized approach to supporting patients who use substances or have a history of substance use, aligning with Island Health’s commitment to person-centred, trauma-informed care.

WHAT YOU NEED TO KNOW

The care plan will be available in two formats:

• Electronic (EHR-integrated) for CPOE-activated sites and select non-CPOE Sites
• Paper-based for select non-CPOE sites

The Substance Use Care Plan aims to:

• Provide standardized documentation and communication across all acute care sites
• Promote person-centred, nonjudgmental care for people who use substances
• Support collaborative care planning and patient support strategies
