New Patient Safety Learning System (PSLS) is Live

Posted On: April 1, 2025

A new cloud-based Patient Safety Learning System (PSLS) has been deployed across Island Health as of March 1, 2025, replacing the old web-based system. Island Health is the third health authority in BC to use this new PSLS. Northern Health has been using it since March 5, 2024, and Interior Health since November 1, 2024.

This new PSLS offers several improvements, including better functionality, new features, and an enhanced user experience.

Key Improvements

  • Integration with the Provincial Patient Registry

You are now able to identify patients using their PHN, then the system will automatically fill in the rest of their information, such as a name and date of birth.

  • Simpler Location Identification

Instead of filling in several different fields, you can simply begin typing the name of your area or unit, then select from the list of options.

  • Streamlined Account Log-in

You are now able to log in to PSLS with your Island Health email address and the same password you use to log in to your work computer. If you're working off site, you'll still need to connect to the network via VPN before logging in to PSLS, to protect the confidentiality and security of electronic data and comply with Island Health's standards for remote access.

Reporting Events in the New PSLS

You can report patient safety incidents in the new PSLS much in the same way you did in the old system. As before, the link to PSLS is available on Island Health's Reporting Intranet Page.

As a reporter, you'll now be able to log in and track your reports—even if you report anonymously. Log-in instructions, along with a variety of resources for learning to navigate the new system, are available on the PSLS Intranet Page. Each report form in PSLS also contains step-by-step instructions and pop-up help.

What Will Happen to the Old PSLS?

The old, legacy system will remain open for the next three months, so PSLS handlers can log in as usual to review and close records that were reported prior to Go Live. After three months, the old PSLS will become a secure, read-only database where the information can be reviewed and downloaded, when needed.

What PSLS Handlers Need to Do

PSLS Handlers with open events in the old PSLS should continue to review those outstanding events to confirm the degree of harm and to document the review activities and actions that have been completed. Once those old events are carefully reviewed, the event status should be changed to Finally Approved.

For the new PSLS, handlers should ensure they have pre-registered for their new PSLS account by following the instructions provided here. Since the new PSLS pulls from Island Health's Global Address List (GAL), Handlers should also ensure their job title and location are accurate. More information on how to ensure your information in the GAL is accurate can be found here.

Resources for handling PSLS events in the new system can be found on the PSLS Tools, Resources & Education intranet page.
