Reporting Incidents & Safety Events!

Located at the top left of the Intranet home page, the new “Report Incidents & Safety Events" button is the place to go to report an incident, patient safety event, or request information/support to address patient/staff health and safety matters. Click the red button to find information and access to:

  • The Provincial Workplace Health Call Centre
    • Staff and providers can report workplace injuries, incidents or exposures.
    • This will commence an incident investigation aimed at preventing a re-occurrence of the incident; as well, it can potentially initiate a WorkSafeBC claim to support staff member(s) while they recover.
  • The Patient Safety & Learning System (PSLS); staff, leaders, and providers can report:
    • Safety events, hazards, or near misses involving PATIENTS or VISITORS
    • Incidents where patients demonstrate aggression towards STAFF, PROVIDERS, and LEADERS
  • The Staff Incident Investigation System
    • Managers/delegates can access staff incident reports and where necessary, initiate an incident review.
  • Critical Incident Stress Management Services
    • Managers/delegates can set up Critical Incident Stress Debriefing (CISD) or other support services for staff.
  • The Emerging Issues Form
    • Directors, Executive Directors or designates can notify senior leadership of emerging issues that require urgent intervention.​
