A strange thing can happen on the road to improvement, fueled by our desire to do better. Wanting better sooner than later, we jump to a solution.

Because we think we know the answer.

Because we're fooled by a symptom of the problem. (e.g. The kids take forever picking and choosing what to pack in lunches, they're late for school, so we reduce their options to speed them up.)

Because we don't understand the root cause of the problem.

Because we don't dig deep enough to determine whether a proposed solution will solve the problem. (e.g. Fewer options speed things up, but dawdling out of bed, the kids continue to be late for school.)
How do we stop jumping to solutions and start landing on solid improvement ideas?
Use the "5 Whys," asking “why" to dig down to the root cause. Start by asking why a problem happens. Then ask why again. Keep asking why until you find the cause, that if addressed, will resolve your problem. There is no magic in the number five. Ask why as many times as needed to find the root cause, and then prepare an impactful improvement idea.

Clinical Governance and QI Support
QI Specialists support C.A.R.E. Networks and Local Levels structures, supporting the prioritization, development and implementation of QI initiatives, including Quality Improvement Plans (QIPs). They also offer QI coaching, at-the-elbow support and training.
