Patient Safety Learning Summaries – Share with your teams to promote & improve patient safety culture

 

Learning Summaries are available to view and share internally within Island Health via the Patient Safety Learning Summaries page!

Learning Summaries (LSs) are developed after one or more Patient Safety Incidents are reviewed through a system-focused lens and are intended to complete the Patient Safety Incident Management (PSIM) cycle by sharing lessons learned and generating discussion about patient safety in your local area. LSs are designed to be engaging one-page, high-level overviews of a patient safety incident(s), the key issues identified and the actions for system improvement. Reviewing these Learning Summaries might help identify new ways to improve practice/care in your program/area.

We now have eight Learning Summaries available, covering a variety of care areas. Some of the topics covered include Heat Wave Impacts on Home Support Clients, Trauma Care, Major Hemorrhage Protocol, Missed Diagnoses, Higher Level of Care (HLOC) Transfers and more. New Learning Summaries will be added consistently over time, so be sure to visit the Learning Summary page regularly to gain ideas on what you and your care team can do to help improve care and patient safety culture!
