The use of correct MHA forms is essential to ensure legislative compliance, accurate documentation and the maintenance of professional standards of care.
Important Update
New and updated Mental Health Act (MHA) Forms go into effect on March 16, 2026.
- Form 7– Application for a Review Panel hearing
- Form 13.1 – Notification to Involuntary Patient of Rights
- Form 14.1 – Notification of Rights to a Patient Under Age 16 admitted at the request of a parent or guardian.
- Form 16.1 & 16.2 – Notices to near relatives related to changes in patient status.
- Form 22 (new) – Request for Independent Rights Advice Service (IRAS) booking
Next Steps for Care Teams
- Access the new and updated forms - available on the BC government website. Use only these forms.
- Remove all outdated MHA form versions from circulation.
- Mental Health Act providers should review the updated forms and familiarize themselves with any new workflow requirements.
