Yarrow Medical, a patient medical home is seeking a NP. The practitioner will be the most responsible primary care provider for a minimum patient panel as described in the NP Primary Care Service Contract template and will work collaboratively with physicians and allied health providers providing primary care to those living in the Downtown and VicWest PCN neighbourhoods.
