We are seeking primary care physicians to staff an innovative new patient stabilization and transition service in Nanaimo.
The service will provide primary care follow-up to patients who have been recently discharged from the emergency department and from inpatient hospital care. The service was developed through a Nanaimo community medical response to help address the needs of unattached patients, and to support the NRGH ED and acute care units.
The service provides access to a course of follow-up care to reduce the risk of admission or re-admission for unattached patients. This much-needed service will support patients with their immediate post-discharge primary care needs, and the team will work to link patients to other services for on-going care and support. Patients referred to the service will be varied from simple follow-up (e.g. cellulitis) to patients with more complex care needs requiring further follow-up and connection to other services. Service will be by referred, booked appointments only.
The role will offer flexible hours, team-based care, and a solid administrative team for support.
- Design your evening and weekend schedule to fit your needs. There is no minimum number of shifts or set schedule.
- Shifts are usually 4 hours
- User-friendly Primary Care EMR - Myle
- Follow-up on lab results will be managed within the team – no need to use your private time
- This is a new service with lots of opportunities to influence clinic design, operations and culture.
Compensation: Group Physician Contract with a rate of $185.77 as of April 1, 2024 (with additional evening increase of $25/hr starting at 18:00)
