Well Planned Action Series: Module 1 – Forming a Foundation!

By the end of Module 1, you will be able to:

  • Identify the common ground that unites the Action Series team to improve the current state
  • Describe how and why a united team approach to medical goals for discharge improves patient care
  • Utilize a communication strategy (e.g. whiteboard) to help engage the broader team

Required Activities:

The first step to building an awesome Action Series team that can work towards improving the experience with medical goals for discharge, is to identify your common ground and to figure out the logistics of doing the Action Series together. The Action Period Worksheet will guide you through four steps that cover the basics.

For change to be successful and spreadable, you need to start by developing awareness with your broader team. Share what you are doing early and often. The monthly Team Activity Update will help you do that!
