Premier announces Island Health Hospital at Home program as provincial prototype

Premier John Horgan and Health Minister Adrian Dix​ today announced that Island Health will lead the province with the introduction of a Hospital at Home prototype at Victoria General Hospital starting next month. The program is a key component of the government's fall pandemic response.

The Hospital at Home program project team continues to plan for the October 2020 launch. We've been reviewing your questions and feedback from our first article and are happy to share more information on our Hospital at Home program and the patient population we will be supporting to start:

Demographics:

  • At least 18 years old
  • Living within geographic catchment area
  • Caregiver in home
  • Safe home environment
  • Phone and refrigerator in home

Clinical Characteristics:

  • Mild to Moderate clinical acuity patients requiring hospital-level care
  • Known diagnosis
  • Time-Limited acute episodes of care (5-7 day length of stay)
  • Able to ambulate without assistance
  • Able to provide self-care, with or without assistance

These demographics and clinical characteristics may change as we evaluate the program. Examples of conditions that will be treated in this program include: congestive heart failure exacerbation, COPD exacerbation, electrolyte abnormalities, cellulitis, pneumonia with hypoxia, anemia, and acute renal injury; however, access to the program will not be limited to specific diagnoses. Based on early modeling as well research from Hospital at Home programs worldwide, it is estimated that between 5-10% of all hospital inpatients would be eligible for the Hospital at Home program.  

The need for community and allied health will be evaluated as the program evolves, however the patient eligibility criteria presently excludes patients who need assistance with self care or mobility. During the planning phase of Hospital at Home, multiple groups within Acute Care and Community Health Services have provided input. Multi-stakeholder involvement has been vital to our work thus far and we look forward to continued collaboration as Hospital at Home evolves.
