The Cowichan Primary Care Network is seeking a nurse practitioner to work with a team of practitioners and other health providers to collectively deliver care to patients at the Cowichan Central Duncan Attachment Clinic. 

Located within the heart of the Cowichan Valley, in central Duncan on Vancouver Island BC, this clinic acts as a hub for team-based care and attachment; and is substantially interconnected with existing primary care practices across the region. This position will provide regular primary care with a focus on mental health and substance use, and will provide care both in the clinic and in the community as needed.

In this service contract position the Nurse Practitioner will participate as a member of an inter-professional team that includes physicians, registered nurses, social workers and behavioral health consultants; supported by a medical office assistant.

Acting as the most responsible primary care provider, there is a requirement for the practitioner to attach 500 patients in their panel by the end of year 3.

After hours’ care for patients is inherent in the services. The nurse practitioner will participate in an after-hours call group, sharing after-hours coverage with other practitioners, to include physicians and nurse practitioners, at the clinic. Schedules are managed mutually amongst the practitioners to ensure availability and continuity of care for patients.

Shift/Hours of Work
The Clinic operates Mon – Fri 09:00-17:00. Full time is 1680 hours of direct care over a minimum of 220 days. After hours care for patients attached to practitioner is inherent in the services. Member of on-call group; expected call rotation

Required Qualifications

Full license/registration as a Nurse Practitioner with the BC College of Nursing Professionals (BCCNP)     

Desired Qualifications

  • Experience and knowledge regarding caring for Indigenous patients. 
  • Experience with medically complex patients experiencing mental health, comorbidity, and mobility challenges.
  • Experience using electronic health record.
  • Experience in team-based care
