While we have been accepting referrals for the Specialist Memory Clinic (SMC) for the last few years, we are proud to announce that the SMC referral guidelines and decision support tools are now live on the Intranet and Pathways for clinicians.
The Specialist Memory Clinic (SMC) is a comprehensive cognitive service for people with memory concerns and/or dementia. The SMC is specialist-driven, embedded within an interdisciplinary team at the Seniors Outpatient Clinic (SOPC).
The SMC enables better management of neurocognitive disorders, facilitated through streamlined diagnosis protocols, comprehensive cognitive assessments and multi-disciplinary team care planning and intervention. The SMC provides earlier and easier access to specialized memory care to ensure more patients and their families are receiving the right care, at the right time, in the right place.
The SMC also embeds research into health care, and provide opportunities for patients to participate in cutting-edge clinical cognitive research at locations beyond the SMC.
